Cancellation Policy
All appointments require a 24 hour cancellation. Please provide 24 hours notice for all canceled appointments, this allows us to see patients that otherwise would not have been able to schedule an appointment. If a 24 hour notice is not given you will be charged a cancellation fee of $25.00 M-F and $35 on Saturdays. Saturday appointments must be canceled by 4pm on Friday. Patients who repeatedly miss appointments may be dismissed from the practice.
Prescription Refills
All medication refills require an appointment. You must make sure you have enough refills on your medication between follow up office visits. D.R. Medical Care policy is to not refill medications at night or on the weekend unless it is an urgent situation. Please plan ahead, so you will not run out of medication.
Out of stock natural supplements must be paid in full prior to order.
Care Management
Our staff will make every effort to return your online requests and phone calls within 48 hours. Please understand that our medical assistants are busy assisting the providers and scheduled patients.
Processing Fees
Please allow seven (7) business days for medical records or other form requests.
Records and forms fees include:
$15 processing fee (which includes postage) plus
$1 per page for copies up to 25 pages
25 cents per page for 26 or greater
$25 minimum for the following forms (FMLA, Special Forms, Medical Reports, Physical Forms, Disability Forms)
Payment Policy
If you are responsible for a co-payment, coinsurance, deductible or entire balance, you will be required to pay at the time services are rendered.
